Job summary

Gentiva is seeking an Afterhours RN Care Manager to deliver compassionate hospice care outside standard hours in Roanoke, VA. You will assess patients, manage plans of care, and coordinate with the IDG for seamless care and timely family communication.

This role emphasizes leadership, clinical oversight, and efficient documentation, including admissions, routine visits, and live discharges while upholding hospice philosophy and compliance.

Qualifications

• Graduate of an accredited nursing program required.
• RN license in the state of employment required.
• Minimum one year RN experience in acute care, oncology, long-term care, hospice, or home health.

Responsibilities

• Assess and coordinate patient and family needs during after-hours shifts.
• Provide direct in-person patient care during on-call, weekend, and after-hours shifts.
