Job summary

Amedisys Admissions Registered Nurse is a full-time role delivering start-of-care assessments and creating comprehensive plans of care for patients in their homes. The RN coordinates with staff, physicians, families, and community resources to ensure timely, high-quality patient care and smooth handoffs.

Responsibilities include OASIS assessments, skilled nursing per plan, patient education, referrals, and promoting health through teaching and rehabilitation.

Qualifications

• Current RN license in the relevant state(s).
• One year of clinical RN experience (or approval by VP of clinical if less).
• Current CPR certification.
• Valid driver's license, reliable transportation and insurance.

Responsibilities

• Performs start-of-care assessment and develops/implements plan of care.
• Completes OASIS assessments timely and accurately.
• Provides skilled nursing care per plan and agency policies.
• Communicates with staff, physicians, families and resources to ensure continuity.
• Initiates referrals when indicated.
• Promotes patient health through teaching and rehabilitation.
• Assists patients in learning self-care techniques.
• Other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of clinical experience as an RN. If less than one year clinical experience, you must be approved by VP of clinical.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
