Job summary

Amedisys Admissions Registered Nurse role involves performing patient start of care assessments, evaluating and implementing the plan of care, and completing OASIS assessments accurately to ensure high-quality home health care.

As part of a full-time team, you will provide skilled nursing care, coordinate with physicians and families, initiate referrals, educate patients on self-care, and maintain continuity of care across the service area.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of clinical experience as an RN. If less than one year clinical experience, you must be approved by VP of clinical.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care.
• Completes OASIS assessments timely and accurately.
• Provides skilled nursing care to patients in accordance with the plan of care and agency policies.
• Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.
• Initiates appropriate referrals when indicated.
• Promotes and maintains patient health through teaching and appropriate rehabilitative measures.
• Assists patients in learning appropriate self-care techniques.
• Other duties as assigned.
