Job summary

Amedisys Home Health is seeking an admissions registered nurse for full-time day shifts in Rhode Island. You will perform start-of-care assessments, develop plans of care, and ensure timely OASIS completion while delivering skilled nursing in patients' homes.

You will educate patients, coordinate with staff and physicians, and initiate referrals as needed to maintain high-quality continuity of care. A $10,000 sign-on bonus is available.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of clinical experience as an RN. If less than one year clinical experience, you must be approved by VP of clinical.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care.
• Completes OASIS assessments timely and accurately.
• Provides skilled nursing care to patients in accordance with the plan of care and agency policies.
• Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.
• Initiates appropriate referrals when indicated.
• Promotes and maintains patient health through teaching and appropriate rehabilitative measures.
• Assists patients in learning appropriate self-care techniques.
• Other duties as assigned.
