Job summary

Amedisys in Lincoln, Nebraska is hiring an admissions registered nurse to perform start of care assessments, develop plans of care, and ensure timely OASIS completion for home health patients.

Qualifications

• Current RN license, state(s) you are assigned to work
• One year of clinical RN experience; if <1 year, approval by VP of clinical
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsibilities

• Performs start of care comprehensive assessment, evaluates and plans care
• Completes OASIS assessments timely and accurately
• Provides skilled nursing care per plan and agency policies
• Maintains continuity of patient care by communicating with staff, physicians, family and resources
• Initiates referrals when indicated
• Promotes health through teaching and rehabilitative measures
• Assists patients in learning self-care techniques

Responsibilities

• Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care.
• Completes OASIS assessments timely and accurately.
• Provides skilled nursing care to patients in accordance with the plan of care and agency policies.
• Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.
• Initiates appropriate referrals when indicated.
• Promotes and maintains patient health through teaching and appropriate rehabilitative measures.
• Assists patients in learning appropriate self-care techniques.
• Other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of clinical experience as an RN. If less than one year clinical experience, you must be approved by VP of clinical.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
