Job summary

Andwell Health Partners is seeking a Hospice Admission RN to deliver skilled nursing services in patients' homes across Scarborough, ME, under physician-approved plans of care.

You will develop care plans with measurable outcomes, educate patients and families, and assess holistic health needs while maintaining clinical competence in pain and symptom management.

Qualifications

• Must hold current Maine RN license.
• ANA certification in Home Health or Community Health Nursing encouraged.
• Current CPR certification required.
• One year of recent clinical nursing experience required.
• Own reliable car with insurance.

Responsibilities

• Provide skilled nursing services in the patient's home per physician plan of care.
• Develop care plans with measurable outcomes in collaboration with patient/family.
• Educate and promote patient rights and informed decision-making in the plan of care.
• Assess and identify the patient’s holistic health care needs.
• Demonstrate competency in assessment and technical skills.
• Apply knowledge of pain control and symptom management.

What You’ll Do

• Develop plans of care with realistic/measurable outcomes in collaboration with the patient/family.
• Provide education and promote patient’s rights and responsibilities for informed decision-making in the plan of care.
• Assess and identify the patient’s holistic health care needs.
• Demonstrate competency in assessment and technical skills.
• Possess and utilize knowledge of pain control & symptom management.

What You’ll Bring

• Current R.N. license in the State of Maine.
• ANA certification in Home Health or Community Health Nursing is encouraged.
