Job summary

Andwell Health Partners in Scarborough, ME is seeking a Registered Nurse to provide skilled nursing in patients' homes under a physician's plan of care.

Develop care plans with measurable outcomes, educate patients and families, assess holistic needs, and demonstrate competency in assessment, pain control, and symptom management. Requires Maine RN license, CPR, and 1 year of clinical experience; a reliable car is needed.

Qualifications

• Current RN license in the State of Maine.
• ANA certification in Home Health or Community Health Nursing encouraged.
• Current CPR card required.
• One year of recent clinical nursing experience required.
• Reliable car with insurance.

Responsibilities

• Provide skilled nursing services to the patient in their own home under the physician's plan of care.
• Develop plans of care with realistic/measurable outcomes in collaboration with the patient/family.
• Provide education and promote patient’s rights and responsibilities for informed decision making in the plan of care.
• Assess and identify the patient’s holistic health care needs.
• Demonstrate competency in assessment and technical skills.
• Possess and utilize knowledge of pain control & symptom management.
