Job summary

Andwell Health Partners is seeking an RN for home health in Scarborough, ME. You will provide skilled nursing in patients’ homes, guided by physician-approved care plans, and collaborate with families to achieve measurable outcomes.

The role supports flexible scheduling and autonomous work in community settings. Applicants should hold an active Maine RN license, CPR certification, and have at least one year of clinical nursing experience.

Qualifications

• Provide skilled nursing in the patient’s own home under physician’s plan of care.
• Develop plans of care with measurable outcomes with patient/family.
• Provide education and promote patient rights in the plan of care.
• Assess patient’s holistic health care needs.
• Demonstrate competency in assessment and technical skills.
• Knowledge of pain control & symptom management.

Responsibilities

• Develop and implement care plans with measurable outcomes in collaboration with patient/family.
• Educate patients and families about health care decisions and plan of care.
• Assess holistic health needs and coordinate with care team.

What You’ll Do:

• Provide skilled nursing services to the patient in their own home under the direction of a physician’s plan of care.
• Develop plans of care with realistic/measurable outcomes in collaboration with the patient/family.
• Provide education and promote patient’s rights and responsibilities for informed decision making in the plan of care.
• Assess and identify the patient’s holistic health care needs.
• Demonstrate competency in assessment and technical skills.
• Possess and utilize knowledge of pain control & symptom management.

What You’ll Bring:

• Current R.N. license in the State of Maine.
• ANA certification in Home Health or Community Health Nursing is encouraged.
• Current CPR card required.
• One year of recent clinical nursing experience is required.
• A reliable car with insurance.
