Job summary

Andwell Health Partners is hiring a Hospice Admission RN to provide skilled nursing services in patients\' homes within the Augusta area. You will develop plans of care, educate patients and families, and assess holistic health needs under a physician's plan of care.

The role requires an active Maine RN license, CPR, and at least 1 year of nursing experience, with a reliable car and insurance. Flexible scheduling and comprehensive benefits are offered.

Qualifications

• Current RN license in Maine is required.
• CPR certification required.
• One year of nursing experience in home care settings preferred.
• Reliable car with insurance is mandatory.

Responsibilities

• Provide skilled nursing services to patients in their homes under physician plans of care.
• Develop and update plans of care with measurable outcomes.

What You’ll Do:

• Provide skilled nursing services to the patient in their own home under the direction of a physician’s plan of care.
• Develop plans of care with realistic/measurable outcomes in collaboration with the patient/family.
• Provide education and promote patient’s rights and responsibilities for informed decision-making in the plan of care.
• Assess and identify the patient’s holistic health care needs.
• Demonstrate competency in assessment and technical skills.
• Possess and utilize knowledge of pain control & symptom management.

What You’ll Bring:

• Current R.N. license in the State of Maine.
• ANA certification in Home Health or Community Health Nursing is encouraged.
• Current CPR card required.
• One year of recent clinical nursing experience is required.
• A reliable car with insurance.
