Job summary

Trustbridge Hospice, part of Empath Health, seeks an RN Hospice Case Manager to coordinate patient care in Central Palm Beach County. This PRN role supports the interdisciplinary team, ensuring comfort and dignity for patients and families while applying hospice principles across care planning and delivery.

Florida-licensed RN with BSN preferred and hospice certification preferred is welcomed, along with BLS and EMR experience.

Qualifications

• Registered Nurse licensed in Florida.
• BSN preferred.
• Hospice certification preferred.
• Current Basic Life Support certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing, or Hospice nursing preferred.
• Understanding of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills, and needs.
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable transportation with valid Florida driver's license.

Responsibilities

• Coordinate patient care under the direction of the primary physician and RN.
• Identify patient/family needs and plan individualized care.
• Collaborate within the interdisciplinary group to implement care plans.
• Utilize monitoring and teaching skills to maximize comfort and quality of life.
• Provide compassionate, personalized service to patients and families.
• Maintain confidentiality and address concerns promptly.

Trustbridge Hospice, a member of Empath Health, is currently seeking a caring and motivated RN Hospice Case Manager to join our team in Central Palm Beach County. You would work the following schedule: PRN 8:30 a.m.- 5 p.m., with weekend availability Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. What you'll Do

• Support and carry out the organization’s mission, vision, and values on a consistent basis.
• Coordinates the care for the patient/family under the direction of the primary physician and registered nurse.
• Responsible for identifying patient/family needs.
• Coordinating the care for the patient/family.
• As part of the interdisciplinary group (IDG), contributes to and follows an individualized plan of care.
• Utilizes monitoring, intervention, and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit.
• Pledge to make the comfort and care of patients and families the highest priority.
• Consistently provide a high level of personalized, sincere, and engaging service to patients and families at all times.
• Ensure the complete satisfaction of patients and families; promptly and effectively address all concerns.
• Maintain utmost respect for confidentiality in the care of the patient/family.

What You’ll Need

• Registered Nurse currently licensed in the State of Florida.
• BSN preferred.
• Hospice certification preferred.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing, or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills, and needs.
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
