The ideal candidate holds an active Florida RN license, BLS, and has experience in hospice, geriatrics, or medical-surgical nursing.

Qualifications

• Hospice/public health/geriatric/medical-surgical experience preferred.

Responsibilities

• Coordinate patient and family care with the primary physician and IDG.
• Conduct thorough assessments of patient and family needs.
• Ensure timely and coordinated delivery of interventions per the care plan.
• Monitor patient conditions and provide interventions and education to enhance comfort.
