Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Registered Nurse (RN) to provide rapid response hospice field visits throughout Palm Beach County. You will assess, triage, and manage urgent conditions with autonomy, collaborating with physicians and the interdisciplinary team to ensure timely, high-quality care.

Florida RN license and BLS required; hospice/public health experience preferred.

Qualifications

• Current Florida RN license and CPR/BLS certification.
• At least 1 year of hospice/public health or related nursing experience preferred.
• Excellent communication and crisis intervention skills.

Responsibilities

• Assess, triage, and manage urgent conditions in non-clinical settings.
• Autonomously coordinate care with physicians and the interdisciplinary team.
• Advocate for patients and families to ensure needs are met.
• Provide education to patients and families to improve comfort and quality of life.
• Utilize EMR to document and coordinate care.

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to provide rapid response hospice field visits throughout Palm Beach County. You will work the following schedule: every Fri/Sat/Sun from 7am - 7:30pm Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.

What you'll Do

• Be responsible for assessing, triaging, and managing urgent and emergent health conditions outside of traditional clinical environments.
• You will operate with a high level of autonomy while collaborating with physicians, advanced practice providers, and interdisciplinary teams to deliver timely, high-quality care and prevent unnecessary emergency department visits or hospitalizations.
• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life.

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs.
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
