Job summary

Trustbridge Hospice is seeking a compassionate Registered Nurse to join our Admissions Team serving Broward County. This full‑time role involves educating families, coordinating care plans, and acting as a liaison among hospitals and healthcare professionals. The weekend schedule is Friday–Sunday, 9:00 am–9:30 pm, with a strong emphasis on empathy and clear communication.

Florida RN license required; BSN preferred and hospice certification preferred. BLS certification must be current.

Qualifications

• Florida RN license; BSN preferred.
• Current BLS certification.
• Experience in hospice, public health, or geriatrics preferred.
• Strong communication and teaching skills with patients and families.
• Proficient with EMR and basic computer skills.

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to join our Admissions Team serving Broward County.

This is a meaningful opportunity for an RN who is passionate about guiding patients and families through one of life’s most significant transitions with empathy, clarity, and dignity.

You will work the following schedule: Friday/Saturday/Sunday 9am - 9:30pm

What you'll Do

• Serve as the primary nurse responsible for evaluating patients referred for hospice services.
• Educate patients and families on hospice care and clearly communicate all components of our continuum of care.
• Collaborate with referring physicians and liaisons to coordinate individualized, hospice‑appropriate plans of care.
• Act as a key liaison between Trustbridge Hospice and hospitals, physicians, long‑term care facilities, and other healthcare professionals.
• Promote and advocate for the hospice philosophy through education and relationship‑building within the healthcare community.

Your work ensures patients receive timely, compassionate care while helping families feel informed, supported, and confident in their hospice journey.

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• BSN preferred Hospice certification preferred.
• Current Basic Life Support Certification.
• Possess the ability to assess physical and psychosocial needs of patient and family.
• Minimum of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing, case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
