Job summary

Tidewell Hospice, part of Empath Health, invites an RN to join our Bradenton, FL team as a PRN RN Case Manager. You’ll provide symptom relief and palliative care under physician oversight, and collaborate with the Interdisciplinary Group to tailor the Plan of Care.

This role focuses on comfort, dignity, and support for patients and families in a community Hospice setting. Required are an approved nursing program graduate with a Florida RN license and CPR certification; hospice experience

Qualifications

• Graduate of approved nursing program with Florida RN license.
• Current CPR certification required.
• At least 1 year of acute-care hospital nursing experience; hospice RN preferred.
• CHPN certification encouraged within three years of employment.
• BSN or MSN preferred.

Responsibilities

• Assess and manage patient symptoms under physician supervision.
• Perform initial and ongoing assessments of the terminal diagnosis impact.
• Develop and update the individualized plan of care with IDG; initiate referrals.
• Educate patients, families, and caregivers about disease, care, and safety.
• Coordinate with the team to arrange needed services.
