Job summary

firsthand Health in Richmond, VA, is seeking a High Intensity Case Manager (RISE Guide) to support individuals with SMI/SUD in a high-touch, hybrid model (in-person and virtual). You will lead intensive care coordination, post-discharge management, and barrier navigation in collaboration with clinicians and peer specialists.

The role emphasizes empathy, trauma‑informed peer support, and direct community engagement (80%+ of time).

Qualifications

• Lived experience with SMI and/or SUD and recovery.
• Two years demonstrated recovery time from mental health and/or substance use.
• Willingness to complete paid training and pass assessments.
• Interest in CHW credential after hire, or already holds certification.
• Training in de-escalation techniques or willingness to be trained.
• Experience with direct in-person engagement for SMI/SUD.
• Knowledge of recovery education and coaching.
• Understanding of whole health and trauma-informed peer support.
• Access to a valid driver's license and ability to drive

Responsibilities

• Provide intensive post-discharge management and transitions of care.
• Coordinate Substance Use Disorder treatment plan where applicable.
• Support medication adherence and communication with prescribing team.
• Develop safety and action planning for deterioration triggers.
• Co-navigate barriers to access care, including transportation and enrollment.
• Problem-solve housing, food, and other social challenges.
• Participate in daily huddles and regular IPRs for coordinated care.
• Possible after-hours rotation for crises outside standard hours.
• Maintain engagement with empathy and proactive problem solving.
• Help prevent sentinel events and improve clinical outcomes.
