Job summary

Connections Health Solutions is seeking a RISE Case Manager to connect clients to resources supporting stabilization related to homelessness, mental health, and substance use disorders. You will engage with clients and families, coordinate crisis and outpatient services, and collaborate with a multidisciplinary team and law enforcement to develop individualized care plans.

You will document services in the medical record, participate in supervision, and support discharge planning.

Qualifications

• High School Diploma or equivalent.
• Experience in a community based or crisis healthcare setting.
• CPR/First Aid certified.
• Valid driver’s license.
• Arizona Level I Fingerprint Clearance Card.
• Fully vaccinated or exempt per policy.
• Own vehicle in good condition.

Responsibilities

• Intervene in community crisis situations with patients, families, and Law Enforcement.
• Assess psychosocial, cultural, emotional, and economic needs during crisis.
• Collaborate with treatment teams and community agencies to address crisis needs.
• Identify gaps in service delivery and recommend discharge plans.
• Maintain accurate clinical records and ensure timely signatures when required.
• Engage in supervision and ongoing education to maintain competency.
• Coordinate re-engagement with clients to reconnect them to care.
