Job summary

Firsthand is seeking a dedicated RISE Guide to provide intensive, community-based support to individuals with SMI/SUD. The role emphasizes empathy, coordination of care, and hands-on problem solving in a high-touch setting.

You will work closely with clinicians and peers to manage post-discharge transitions, support treatment adherence, and navigate barriers to care. A strong emphasis on safety planning and trauma-informed approaches is required.

Qualifications

  • Lived experience with SMI and/or SUD and recovery.
  • Two years demonstrated recovery time from a mental health and/or substance abuse disorder at the date of application.
  • Agreement to complete a paid training period prior to serving individuals and pass assessments to move forward.
  • Interest in gaining a Community Health Worker (CHW) Credential after hire, or already holds certification.
  • Training or experience in de-escalation techniques (e.g., CPI, Mental Health First Aid) or a willingness to be trained in these methods.

Responsibilities

  • Provide intensive post-discharge management and transitions of care, ensuring stability after psychiatric hospitalization.
  • Coordinate and support Substance Use Disorder treatment plans.
  • Support individuals in navigating medications and addressing adherence barriers.
  • Develop safety and action plans based on individual supports and triggers.
  • Co-navigate systemic barriers to access care, including appointments and transportation.
  • Assist with housing, food, transportation, and other social challenges as needed.
  • Participate in daily huddles and regular IPRs for a coordinated approach.
  • Potentially participate in after-hours rotation for crisis support.
  • Maintain engagement and build trusting relationships with individuals and the care team.
