Job summary

Mosaic Life Care is seeking a Program Coordinator to lead the rural health Hub within the ToRCH Care model. You will organize local implementation, align partners, and manage day-to-day Hub operations across hospitals, FQHCs/RHCs, behavioral health providers, EMS, and community organizations.

The role requires strong collaboration, data-driven reporting, and the ability to coordinate across multiple partners while advancing regional strategies and workflows.

Qualifications

• Associate’s Degree in a related field required.
• Bachelor’s degree preferred for broader leadership responsibilities.
• 1–3 years of experience in healthcare operations, community health, or care coordination.
• Experience building and sustaining partnerships across healthcare providers, EMS, public health agencies, and community organizations.

Responsibilities

• Serve as the primary Hub spokesperson and coordinate partners to advance Hub priorities.
• Convene Hub members to assess local needs, set priorities and develop a tailored Hub strategy.
• Oversee localized program implementation, budgets, and timeline coordination across partners.
• Monitor Hub performance and navigator utilization to improve referrals and workflows.
• Support Leadership Board governance, meeting coordination, and follow-up on operational actions.
• Ensure onboarding and alignment across hospitals, FQHCs/RHCs, behavioral health providers, EMS, and community orgs.
