Job summary
Purchase District Health Department is seeking a dedicated Community Health Worker to support the Rural Community Hubs for Chronic Care Innovation (RCH). The role focuses on recruiting partners, onboarding organizations, coordinating across health, social services, and education sectors, and strengthening referral pathways within the Purchase Area Development District.
You will gather partner input, contribute to Hub planning, and help reflect organizational capacity and gaps to Hub leadership.
Qualifications
- Community Health Worker certification or eligibility preferred.
- Experience serving rural communities preferred.
- 4+ years of experience in community health work or related roles.
- Bachelor's degree or equivalent lived/professional experience preferred.
Responsibilities
- Organizational Engagement: Build relationships with partners, raise awareness of Hub services.
- Partner Coordination: Help connect services to health care, social services, and community resources.
- Relationship Building: Collaborate with local organizations and leaders for cross-sector work.
- Documentation & Insight: Track outreach and partner engagement for reporting and improvement.
