Job summary

Mercy is seeking a Rural Health Transformation Program Coordinator to lead an integrated hub, oversee Navigator staff, and drive program performance. You will implement governance support, coordinate across counties, and ensure alignment with Mission leadership and Hub Boards.

Responsibilities include providing operational leadership, stakeholder facilitation, and building partnerships with regional networks to advance rural health initiatives and program outcomes.

Qualifications

• Bachelor’s degree required in Public Health, Social Work, Healthcare Administration, Community Health, or related field.
• Three to five years of experience in community health, program coordination, healthcare outreach, or related field.
• Demonstrated leadership, supervisory, or team coordination experience.
• Strong communication, facilitation, organizational, and project management skills.

Responsibilities

• Provide operational leadership for a Rural Health Transformation Program hub and supervise Navigator staff.
• Serve as the primary liaison between Mission leadership, community stakeholders, Hub Boards, and the Regional Coordinating Network.
• Coordinate governance support, program performance, and multi-site community health initiatives.
