Complete Care - Illinois is seeking a Restorative Nurse (RN/LPN) to provide direct nursing care while leading the facility's Restorative Nursing Program, focusing on resident function and preventing decline.

The role requires accurate MDS documentation, supervision of nursing assistants, collaboration with therapists and physicians, and compliance with federal, state, and local regulations.

Qualifications

• Must hold current Illinois RN or LPN license.
• Experience with MDS documentation and restorative care preferred.
• CPR certification required.
• RAC-CT certification preferred.
• Knowledge of long-term care regulations.
• Strong communication and supervisory skills.

Responsibilities

• Direct, develop, and supervise the Restorative Nursing Program and train nursing staff.
• Collaborate with therapists, nurses, and physicians to set SMART restorative goals.
• Maintain accurate daily MDS-related documentation and coding.
• Participate in care plan meetings with input on functional status and independence.
• Audit restorative documentation for accuracy and regulatory compliance.
• Provide leadership and supervision of nursing personnel on your unit/shift.
