Elevate Care Des Plaines is hiring a Restorative Nurse (RN or LPN) to enhance residents' quality of life through restorative care and rehab. You will develop and implement personalized care plans to help residents reach their highest level of independence.

Responsibilities include creating restorative plans, monitoring progress, guiding therapeutic activities, educating residents and families, and documenting progress. Competitive pay and benefits offered.

Qualifications

• Active RN or LPN license in good standing.
• Experience in restorative nursing, rehab, or long-term care preferred.
• Strong assessment and critical thinking skills.
• Excellent communication and teamwork abilities.

Responsibilities

• Develop and implement restorative care plans to promote independence.
• Monitor and evaluate resident progress and adjust plans as needed.
• Provide therapeutic activities to maintain or improve function.
• Educate residents and families on exercises and safety strategies.
• Collaborate with nursing and therapy teams for coordinated care.
• Document and record resident progress accurately.
