Restorative Nurse (LPN / RN)

SUMMARY:

The Restorative Nurse is a critical member of the clinical team, responsible for providing direct nursing care while simultaneously spearheading and supervising the facility's Restorative Nursing Program (RNP). This role requires a focus on maintaining resident function, preventing further decline, and ensuring accurate documentation related to Minimum Data Set (MDS) coding. The supervision of nursing activities, particularly those related to restorative care, must be in accordance with current federal, state, and local standards, guidelines, and regulations to ensure the highest degree of quality care is maintained at all times.

ESSENTIAL DUTIES AND RESPONSIBILITIES (Restorative & Clinical Focus):

Restorative and MDS Responsibilities:

• Program Oversight: Direct, develop, and supervise the day-to-day functions of the Restorative Nursing Program, training nursing assistants on specific restorative techniques (e.g., ambulation, range of motion, feeding).
• Assessment & Goal Setting: Collaborate with therapists, nurses, and physicians to identify residents who would benefit from RNP services. Set and monitor specific, measurable, achievable, relevant, and time-bound (SMART) restorative goals for each participating resident.
• MDS Focus: Perform and/or assist with assessments and accurate daily documentation that supports the Restorative Program to ensure appropriate coding on the Minimum Data Set (MDS).
• Interdisciplinary Planning: Participate in care plan meetings, providing valuable input on the resident's functional status, restorative progress, and potential for independence.
• Quality Assurance: Audit and monitor restorative documentation completed by nursing assistants for accuracy, compliance, and proper reflection of the care provided.

General Clinical Responsibilities:

• Supervision: Provide leadership to nursing personnel assigned to your unit/shift. Direct the day-to-day functions of the nursing assistants, meeting with them to plan the shift's services and activities.
• Documentation: Complete & file required recordkeeping forms/charts upon the resident's admission, transfer, and/or discharge. Chart nurse's notes in an informative & descriptive manner that reflects the care provided and the resident's response.
• Order Management: Receive & transcribe telephone orders from physicians, record them on the Physician's Order Form, and notify appropriate departments.
• Medication & Treatment: Prepare & administer medications as ordered by the physician. Administer professional services such as catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, as required. Provide IV & IV site care as needed.
• Communication: Receive/give the nursing report upon reporting in and ending shift duty hours. Make rounds with physicians as necessary.

QUALIFICATIONS:

To perform this job successfully, an individual must demonstrate proficiency and possess the following:

• License: Must possess a current, unencumbered, active license to practice as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Illinois.
• Experience (Preferred): MDS/Restorative experience is highly preferred.
• Certification (Preferred): RAC-CT (Resident Assessment Coordinator-Certified) certification is strongly preferred.
• Knowledge: Must be knowledgeable of nursing & medical practices & procedures, as well as laws, regulations, and guidelines that pertain to long-term care facilities.
• Clinical Skills: Must be able to read, write, speak, and understand the English language, and possess the ability to make independent clinical decisions when circumstances warrant such action.
• Certifications (Required): Current CPR Certification is required.
• Experience: Experience in Long Term Care is preferred.
