Qualifications

• Must hold a current, active RN or LPN license in Illinois.
• MDS/restorative nursing experience is highly preferred.
• RAC-CT certification is strongly preferred.
• Knowledge of long-term care laws, regulations, and best practices.
• CPR certification is required.
• Ability to read, write, speak, and understand English; sound clinical judgment.

Responsibilities

• Direct and supervise the Restorative Nursing Program and train nursing assistants.
• Collaborate with therapists, nurses, and physicians to set SMART restorative goals.
• Ensure accurate MDS-related documentation and coding support.
• Lead care plan meetings and contribute to interdisciplinary discussions.
• Audit restorative documentation for accuracy and compliance.
