Elevate Care is hiring a Restorative Nurse (RN or LPN) in Illinois to develop and implement restorative care plans that promote resident independence. You will monitor progress, provide therapeutic activities, and educate residents and families while collaborating with nursing and therapy teams.

Qualifications

• Active RN or LPN license required.
• Experience in restorative nursing, rehab, or long-term care preferred.

Responsibilities

• Develop and implement restorative care plans with residents and care teams.
• Monitor and evaluate resident progress; adjust plans as needed.

• Educate residents and families on exercises, safety, and recovery strategies.
• Collaborate with nursing and therapy teams for coordinated care.
• Document and record resident progress accurately.
