The role requires an active IL license (RN or LPN) and CPR certification, with preferred MDS/restorative experience and RAC-CT certification.

You will supervise nursing assistants, coordinate care plans, and ensure compliance with federal, state, and local regulations

Qualifications

• Active Illinois RN or LPN license.
• CPR certification is required.
• MDS/restorative experience is preferred.
• Knowledge of long-term care regulations and standards.
• Strong communication and leadership abilities.

Responsibilities

• Lead and supervise Restorative Nursing Program activities.
• Assess residents and set SMART restorative goals with care team.
• Ensure accurate daily documentation and MDS coding support.
• Coordinate with therapists, nurses, and physicians during care planning.
• Provide skilled nursing care and supervise nursing staff on unit.
