Job summary

Pivot is seeking a Restorative Aide to support restorative and rehabilitative care for ProCare I-SNP members. Responsibilities include assisting with personal care, restorative tasks, and promoting patient independence within policy guidelines.

Candidate must hold a Nursing Assistant certification, be fluent in English, and be available six days weekly. Travel to multiple SNF locations within the area may be required; Pivot offers a comprehensive benefits package and equal opportunity employment.

Qualifications

• State certification as a Nursing Assistant is mandatory.
• Ability to read, write, and speak the English language.
• Must be available to work six days per week.
• Ability to travel to additional SNF locations within geographical area.

Responsibilities

• Provide personal care to patients ensuring safety and comfort.
• Assist in restorative or rehabilitative care.
• Perform restorative nursing tasks (ambulation, ROM, positioning, self-feeding) within training scope.
• Promote independence for patients.
• Advises Director of Rehab of patient status daily.
• Contributes to patient care plans with knowledge of conditions.
• Assist in implementing care plans per established policy and procedure.
• Observe and report patient conditions and document observations.

Job description

The overall purpose of the Restorative Aide position is to support in the provision of restorative and/or rehabilitative care of ProCare I-SNP members as appropriate.

• Provide personal care to patients in a manner conducive to their safety and comfort. Assist in the provision of restorative or rehabilitative care.
• Performs restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc., within the scope of individuals' training.
• Promotes and supports the greatest possible degree of independence for patients.
• Advises Director of Rehab (DOR) of Patient's status daily.
• Contributes knowledge of Patient's conditions/needs to patient care plans.
• Assists in the implementation of care plans as directed within the parameters of established policy and procedure.
• Observes and reports on Patients' conditions and documents observations as required.
