Job summary

Pivot in San Antonio, TX is seeking a Restorative Aide to support restorative and rehabilitative care for ProCare I-SNP members. Responsibilities include personal care, restorative tasks, promoting independence, and daily status updates to the Director of Rehab.

The role requires CNA certification, English fluency, ability to travel between SNF locations, and willingness to work six days a week. Pivot offers a comprehensive benefits package including 401K, CEU reimbursement, and health plans.

Qualifications

Responsibilities

• Contributes knowledge of patient conditions/needs to patient care plans.

The overall purpose of the Restorative Aide position is to support in the provision of restorative and/or rehabilitative care of ProCare I-SNP members as appropriate.

• Provide personal care to patients in a manner conducive to their safety and comfort. Assist in the provision of restorative or rehabilitative care.
• Performs restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc., within the scope of individuals' training.
• Promotes and supports the greatest possible degree of independence for patients.
• Advises Director of Rehab (DOR) of Patient's status daily.
• Contributes knowledge of Patient's conditions/needs to patient care plans.
• Assists in the implementation of care plans as directed within the parameters of established policy and procedure.
• Observes and reports on Patients' conditions and documents observations as required.

Qualifications

• State certification as a Nursing Assistant is mandatory.
• Ability to read, write, and speak the English language.
• Must be available to work six days per week
• Ability to travel to additional SNF locations within geographical area
