Job summary

Pivot Rehabilitation Services in the San Antonio area is seeking a Restorative Aide to support restorative and rehabilitative care for ProCare I-SNP members. You will perform CNA tasks under supervision, focusing on safety, comfort, and preserving patient independence.

Responsibilities include ambulation, range of motion, positioning, self-feeding, and contributing to care plans. The role requires Texas CNA certification, English proficiency, availability six days per week, and the ability to

• Advise Director of Rehab (DOR) of patient status daily and contribute to care plans.
• Assist in implementing care plans within policy and procedures.
• Observe and report patient conditions and document observations as required.

The overall purpose of the Restorative Aide position is to support in the provision of restorative and/or rehabilitative care of ProCare I-SNP membersas appropriate.

• Provide personal care to patients in a manner conducive to their safety and comfort. Assist in the provision of restorative or rehabilitative care.
• Performs restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc., within the scope of individuals' training.
• Promotes and supports the greatest possible degree of independence for patients.
• Advises Director of Rehab (DOR) of Patient's status daily.
• Contributes knowledge of Patient's conditions/needs to patient care plans.
• Assists in the implementation of care plans as directed within the parameters of established policy and procedure.
• Observes and reports on Patients' conditions and documents observations as required.

Qualifications

• State certification as a Nursing Assistant is mandatory.
• Ability to read, write, and speak the English language.
• Must be available to work six days per week
• Ability to travel to additional SNF locations within geographical area
