Job summary

Pivot Rehabilitation Services is seeking a Restorative Aide to support restorative and rehabilitative care for ProCare I-SNP members. You will provide personal care, assist with restorative nursing tasks, and promote patient independence.

Must hold state Nursing Assistant certification, be able to work six days weekly, and travel to multiple SNF locations within the area. Excellent benefits include 401K with match, CEU reimbursement, and comprehensive insurance plans.

Qualifications

• State certification as a Nursing Assistant is mandatory.
• Must be available to work six days per week.
• Ability to travel to additional SNF locations within geographical area.

Responsibilities

• Provide personal care to patients to ensure safety and comfort.
• Assist in restorative and rehabilitative care tasks (ROM, ambulation, positioning, self-feeding).
• Promote independence and patient autonomy to the greatest degree possible.
• Advise the Director of Rehab of patient status daily.
• Contribute knowledge of patient conditions to care plans.
• Assist in implementation of care plans as directed within policy.
• Observe and report patient conditions and document observations as required.

The overall purpose of the Restorative Aide position is to support in the provision of restorative and/or rehabilitative care of ProCare I-SNP members as appropriate.Provide personal care to patients in a manner conducive to their safety and comfort. Assist in the provision of restorative or rehabilitative care.Performs restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc., within the scope of individuals' training.Promotes and supports the greatest possible degree of independence for patients.Advises Director of Rehab (DOR) of Patient's status daily.Contributes knowledge of Patient's conditions/needs to patient care plans.Assists in the implementation of care plans as directed within the parameters of established policy and procedure.Observes and reports on Patients' conditions and documents observations as required.State certification as a Nursing Assistant is mandatory.Ability to read, write, and speak the English language.Must be available to work six days per weekAbility to travel to additional SNF locations within geographical areaDiversity, Equity, and Inclusion are at the heart of Pivot.
