Job summary

Pivot is seeking a Restorative Aide at Ashford Gardens, Houston, TX. This full-time role supports restorative and rehabilitative care for ProCare I-SNP members in a SNF setting.

Responsibilities include personal care, ROM, ambulation, positioning, self-feeding, and promoting patient independence. CNA certification and English fluency are required; travel to other SNF locations within the area may be needed. Pivot offers strong benefits and competitive compensation.

Qualifications

• State certification as a Nursing Assistant is mandatory.
• Ability to read, write, and speak the English language.
• Must be available to work six days per week.
• Ability to travel to additional SNF locations within geographical area.

Responsibilities

• Provide personal care to patients, ensuring safety and comfort.
• Performs restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding.
• Promotes and supports the greatest possible degree of independence for patients.
• Advises Director of Rehab (DOR) of Patient's status daily.
• Contributes knowledge of Patient's conditions/needs to patient care plans.
• Assists in the implementation of care plans as directed within policy and procedure.
• Observes and reports on Patients' conditions and documents observations as required.

Job Location: Pivot at Ashford Gardens - Houston, TX 77076, Position Type: Full Time, Salary Range: Undisclosed, Job Category: Clinical Support, The overall purpose of the Restorative Aide position is to support in the provision of restorative and/or rehabilitative care of ProCare I-SNP members as appropriate.
