Job summary

Cantex Continuing Care Network in San Antonio, TX is seeking a Restorative Aide to support restorative and rehabilitative care for ProCare I-SNP members. This full-time role emphasizes patient safety, comfort, and independence under the supervision of the Director of Rehab.

You will perform restorative nursing tasks such as ambulation, ROM, positioning, and feeding, and contribute to care plans by observing and reporting patient status and needs daily.

Qualifications

• State certification as a Nursing Assistant is mandatory.
• Ability to read, write, and speak the English language.
• Must be available to work six days per week.
• Ability to travel to additional SNF locations within geographical area.

Responsibilities

• Provide personal care to patients, ensuring safety and comfort while assisting with restorative care.
• Perform restorative nursing tasks such as ambulation, ROM, positioning, and self-feeding.
• Promote independence and support care plans under supervision of DOR.
• Observe, document, and report patient conditions as required.
• Communicate patient status to the Director of Rehab daily.

The overall purpose of the Restorative Aide position is to support in the provision of restorative and/or rehabilitative care of ProCare I-SNP membersas appropriate.

• Provide personal care to patients in a manner conducive to their safety and comfort. Assist in the provision of restorative or rehabilitative care.
• Performs restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc., within the scope of individuals' training.
• Promotes and supports the greatest possible degree of independence for patients.
• Advises Director of Rehab (DOR) of Patient's status daily.
• Contributes knowledge of Patient's conditions/needs to patient care plans.
• Assists in the implementation of care plans as directed within the parameters of established policy and procedure.
• Observes and reports on Patients' conditions and documents observations as required.
