Job summary

Pivot Rehabilitation Services is seeking a Restorative Aide to support restorative and rehabilitative care for ProCare I-SNP members. You will provide personal care, assist with restorative nursing tasks, and promote patient independence.

Must hold state Nursing Assistant certification, be able to work six days weekly, and travel to multiple SNF locations within the area. Excellent benefits include 401K with match, CEU reimbursement, and comprehensive insurance plans.

Qualifications

State certification as a Nursing Assistant is mandatory.
Must be available to work six days per week.
Ability to travel to additional SNF locations within geographical area.

Responsibilities

Provide personal care to patients to ensure safety and comfort.
Assist in restorative and rehabilitative care tasks (ROM, ambulation, positioning, self-feeding).
Promote independence and patient autonomy to the greatest degree possible.
Advise the Director of Rehab of patient status daily.
Contribute knowledge of patient conditions to care plans.
Assist in implementation of care plans as directed within policy.
Observe and report patient conditions and document observations as required.
