Job summary

Pivot is seeking a Restorative Aide to support restorative and rehabilitative care for ProCare I-SNP members at Windsor Gardens- Lancaster, TX. The role includes personal care, restorative tasks, and collaboration with the rehab team to enhance patient independence.

You will report observations to the Director of Rehab and assist in implementing care plans while traveling to SNF locations as needed.

Qualifications

• State certification as a Nursing Assistant is mandatory.
• Ability to read, write, and speak the English language.
• Must be available to work six days per week
• Ability to travel to additional SNF locations within geographical area

Responsibilities

• Provide personal care to patients to support safety and comfort.
• Assist in restorative or rehabilitative care as directed.
• Perform restorative nursing tasks (ambulation, ROM, positioning, self-feeding) within training scope.
• Promote independence for patients.
• Advise Director of Rehab of patient status daily.
• Contribute to care plans with knowledge of patient conditions/needs.
• Assist in implementation of care plans and document observations.

The overall purpose of the Restorative Aide position is to support in the provision of restorative and/or rehabilitative care of ProCare I-SNP members as appropriate.

• Provide personal care to patients in a manner conducive to their safety and comfort. Assist in the provision of restorative or rehabilitative care.
• Performs restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc., within the scope of individuals' training.
• Promotes and supports the greatest possible degree of independence for patients.
• Advises Director of Rehab (DOR) of Patient's status daily.
• Contributes knowledge of Patient's conditions/needs to patient care plans.
• Assists in the implementation of care plans as directed within the parameters of established policy and procedure.
• Observes and reports on Patients' conditions and documents observations as required.
