Qualifications

Responsibilities

• Provide guidance, education, supervision, and direct supports to consumers to maintain health and work toward independent living.
• Collaborate with staff to ensure safe, supportive care and crisis or situational stabilization.
• Document consumer progress and activities in program records.
• Support consumers in learning daily living skills and community integration.
