Job summary

ADAPT Community Network is seeking a Residence Program Specialist to support residents in a New York City residential facility. You will provide direct care, assist with daily living activities, and advocate for participant goals to promote independence and quality of life.

Under supervision, you will participate in treatment plans, ensure safety, document activities, and help with meals and transportation as needed.

Qualifications

• High School Diploma or GED required.
• Six months of related experience preferred but not required.
• AMAP designation required at hire or within six months of date of hire.
• Valid New York State Driver’s License in good standing.
• Ability to read, write, speak, and understand English.

Responsibilities

• Provides direct care and supports individuals to achieve greater independence.
• Ensures safety, health, and well-being of residents at all times.
• Assists with activities of daily living and community living skills.
• Maintains a respectful, homelike atmosphere in the residence.
• Documents progress notes and required records accurately and timely.
• Responds to crises and medical emergencies and notifies appropriate personnel.
