Job summary

ADAPT Community Network is seeking a Residence Program Specialist to support individuals living in a residential facility in New York City. The role emphasizes dignity, respect, and person-centered care to help residents achieve their personal goals and the highest quality of life.

You will provide direct support, skill-building, and advocacy, coordinating with nursing and other staff to ensure safety and well-being and supporting independence through daily activities and routine tasks.

Qualifications

• High School Diploma or GED required.
• AMAP eligibility within six months of hire.
• Valid NYS driver’s license in good standing.
• Ability to read, write, speak and understand English.
• Strong interpersonal and communication skills.
• Ability to respond to crisis situations.

Responsibilities

• Provides care and supports individuals to achieve independence and personal development.
• Maintains safety, health, and well-being of residents at all times.
• Teaches ADLs and supports social and community living skills.
• Maintains a homelike, respectful atmosphere in the residence.
• Participates in interdisciplinary treatment plans and implements program goals.
• Documents daily activities, progress notes, and incident reports accurately.
