PathWays of Southwestern Pennsylvania seeks a Program Specialist for our Residential Group Homes to support adults with intellectual disabilities and autism in achieving independence and meaningful daily life.

You will develop ISPs, coordinate routines, track progress, and ensure compliance with state regulations while providing hands-on support when needed. The role requires care for residents, attention to safety, and collaboration with a multi-disciplinary team.

Qualifications

• Prioritizes accuracy in documenting ISPs and progress reports.
• Able to collaborate with interdisciplinary teams to design daily routines.
• Commitment to safeguarding rights and promoting independence for residents.

Responsibilities

• Develop and oversee Individual Support Plans (ISPs) for residents.
• Collaborate with team members to create meaningful daily routines.
• Monitor progress and prepare reports as required by regulations.
• Participate in interdisciplinary team meetings.
• Ensure compliance with ODP, OLTL, and DPW regulations.
• Step in with hands-on support when needed to maintain continuity of care.
