Job summary

Premier Community Supports, LLC. is seeking a Residential Direct Support Professional to assist residents with daily living in a group home in Grants Pass, Oregon. You will support routines, health needs, and community participation to foster independence and safety.

The role emphasizes compassionate, person-centered care, collaboration with families and care teams, and adherence to ISP/behavior plans while flexible scheduling is required across day/evening/overnight shifts.

Qualifications

• Must be at least 18 years old.
• HS diploma or GED required.
• Preferred: 6 months I/DD experience or 1 year caregiving experience.
• Pass criminal background check as required by state (Oregon).
• Valid driver’s license, auto insurance and reliable vehicle if transportation duties apply.
• Smartphone access for documentation and timekeeping.
• CPR/First Aid certification (training provided if needed).
• Willingness to complete required training (24 hours minimum annually).
• Comfort around animals if applicable.

Responsibilities

• Build supportive, person-centered relationships aligned with ISP and goals.
• Promote autonomy, dignity, and rights in daily life.
• Assist with activities of daily living (bathing, dressing, grooming, hygiene, etc.).
• Support household tasks and maintain a safe home-like environment.
• Assist with meals, budgeting, and grocery planning.
• Facilitate community participation and transportation for appointments and activities.
• Monitor health/behavior, document daily notes and incident reports per regulations.
• Collaborate with families and interdisciplinary teams for coordinated care.
• Advocate for individuals' needs and rights; maintain a respectful environment.
