Premier Community Supports LLC is seeking a Residential Direct Support Professional (DSP) to support individuals in a Grants Pass residential setting, focusing on daily living, skills development, safety, and meaningful activities.

You will work as part of a collaborative team, providing person-centered care, documentation, and transportation support while promoting independence and community participation.

Formación

• Minimum 18 years old and High School diploma or GED.
• At least six months of experience supporting adults/IDD or one year with mental health conditions.
• Strong verbal and written communication and thorough documentation.
• Valid driver’s license and reliable vehicle if transportation duties apply.
• Must complete background check per Oregon rules.

Responsabilidades

• Build person-centered relationships and align care with ISP and care plans.
• Assist daily living activities and promote independence.
• Transport residents to appointments and community activities.
• Document daily notes, progress, and incidents accurately.
• Collaborate with families and care teams to ensure coordinated care.
