Job summary

Premier Community Supports, LLC is seeking Direct Support Professionals to assist individuals with IDD in-home and in the community across Lane County, Oregon.

You will support individuals to gain independence, follow their ISP, promote autonomy, handle daily living tasks, and help plan social activities while prioritizing safety and dignity. This role includes transport, documentation, and compliance with regulations, with varying day/evening shifts including weekends.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED required.
• At least six months experience supporting adults/IDD or one year with mental health conditions.
• Strong verbal and written communication skills; accurate documentation.
• Must pass criminal background check per Oregon rules.
• Valid driver's license and reliable vehicle when transportation duties apply.
• Access to a smartphone for timekeeping and communication.
• Current CPR and First Aid certification; training may be provided.
• Willingness to complete annual training (min 24 hours).

Responsibilities

• Build a supportive, person-centered relationship with the individual and deliver care in alignment with the ISP.
• Promote autonomy and rights; help individuals make choices about their lives.
• Assist with daily living activities and household tasks as needed.
• Collaborate with family and support team to achieve best outcomes.
• Facilitate participation in social, recreational, and appointment-based activities.
• Support budgeting, shopping, and meal preparation when needed.
• Administer medications with training; follow medication protocols.
• Identify and report health, behavior, or circumstance changes; maintain documentation.
• Provide transportation for errands, community outings, or appointments.
• Adapt to day, evening, night shifts and weekends as required.
