Qualifications

• High School Diploma required.
• Valid driver's license required (USA).

Responsibilities

• Assess individuals for hopes, dreams, and desires and provide supports to achieve them.
• Support individuals in developing relationships in their community and with natural supports.
• Assist individuals in participating in their community aligned with their interests.
• Protect and promote the rights of individuals receiving services and support advocacy efforts.
• Provide emotional, physical, and medical support as needed.
• Foster person-centered, positive relationships using positive approaches that promote self-determination.
• Contribute to assessment and planning processes and develop individual plans.
• Complete required documentation including goal progress, incident reports, and timesheets.
• Assist with home operations: housekeeping, meals, laundry, shopping, lawn care, vehicle cleanliness, maintenance.
• Drive and arrange transportation as required.
• Educate on emergency procedures and personal safety; conduct drills and safety practices.
• Assist staff with orientation, mentoring, and training.
• Sleepover at residential settings may be required.
• Attend trainings and maintain required certifications.
• Follow agency policies and state/federal regulations; adhere to service definitions.
• Complete other duties as assigned by supervisor.
