Catholic Charities of New York is seeking a Residential Counselor to assist residents in learning community life skills within the Beacon of Hope Congregate Programs. You will implement service plans, provide counseling, and coordinate with medical, psychiatric, and support services to help individuals move toward greater independence.

The role is full-time, on-site at facilities in Bronx or Staten Island, with a schedule that may include emergencies.

Formación

• High School Diploma required.
• Experience helping individuals with serious mental illness or similar population.
• Ability to learn CPR/First Aid and maintain certification.

Responsabilidades

• Provide case management services to residents.
• Implement individual service plans to attain rehabilitation goals.
• Assist residents in developing daily living skills (grooming, dressing, meals, meds, budgeting).
• Document services and interactions in case records and maintain records.
• Engage residents in discussions about goals and service evaluation.
• Provide ongoing counseling support as needed.
• Draft comprehensive service plans and present materials in meetings.
• Coordinate with other services to meet residents' needs (medical, psychiatric, benefits, etc).
• Manage incidents and minimize risks; notify Manager and document events.
• Collaborate with agency staff and external service providers.
