Job summary

Behavioral Health Network, Inc. is seeking a Counselor/Case Manager for its Transitional Support Services (TSS) team in Massachusetts. The role provides counseling, case management, and recovery support to residents, managing caseloads and coordinating discharge planning with outpatient resources.

Qualifications

• High school diploma or GED required; associate degree or higher preferred.
• 1–3 years of experience in a substance-use treatment environment.
• Knowledge of substance-use disorders and recovery-oriented care.
• Strong communication, documentation, and organization skills.
• Familiarity with internet, email, word-processing, and EHR systems.
• Valid driver’s license and ability to complete required driving training.

Responsibilities

• Provide direct counseling, case management, and recovery support to individuals in residential care.
• Manage caseload, meet with individuals as required, and support recovery and community integration.
• Complete admissions/discharges and develop assessments, treatment and safety plans.
• Coordinate discharge and aftercare, linking to outpatient treatment and housing resources.
• Document participation and progress in the electronic health record and participate in MDT meetings.
