Job summary

Behavioral Health Network, Inc. seeks a Counselor/Case Manager to join our Clinical Stabilization Services team. You will provide direct counseling, manage a caseload, and coordinate discharge planning to ensure integrated care and community resource connections.

Applicants should have 1–3 years of substance-use treatment experience and strong interpersonal skills. This role supports 24-hour residential programs and requires HIPAA-compliant documentation and on-call availability.

Qualifications

• High school diploma or GED required; associate degree or higher preferred.
• 1–3 years of experience in a substance-use treatment environment required.
• Knowledge of substance-use disorders, withdrawal and stabilization, recovery-oriented care, case management, treatment planning, relapse-prevention, and community resources.

Responsibilities

• Provide direct counseling, case management, and recovery support to individuals in structured residential treatment.
• Manage an assigned caseload and develop individualized assessments and treatment plans.
• Coordinate continuing-care and discharge planning with outpatient treatment, housing, healthcare, and community resources.
• Document participation and progress in the electronic health record; participate in multidisciplinary teams and on-call support when needed.
