Job summary

High Point & Affiliated Organizations, through its Transitional Support Services (TSS) in New Bedford, seeks a Case Manager to support residents transitioning from detox to step-down services. The role emphasizes intake processing, ISP development, and ongoing progress reviews with residents.

Qualifications include a High School Diploma (or GED) with supervised counseling experience preferred; an Associate's degree or higher is also favored.

Qualifications

• Must present as courteous and professional at all times.
• Excellent communication skills, including clear and organized expression.
• Knowledge of basic computer skills and software programs.
• Ability to work with others in cooperative and collaborative manner.
• Understanding of relationship between substance use and mental health issues.

Responsibilities

• Facilitate intakes of new residents and complete intake paperwork.
• Complete Initial ISP with resident and update ISP monthly.
• Maintain accurate case files and document referrals and authorizations.
• Meet bi-weekly with residents to review progress and adjust goals.
• Attend monthly case reviews and required supervision/training.
• Handle driving/transportation duties and respond to emergencies when needed.
