Job summary

Behavioral Health Network, Inc. seeks a Residential Coordinator to lead and support residents in ACCS program group homes. You will supervise Residential Support Specialist staff and coordinate services to implement ISP goals and objectives.

You will oversee medication treatment, crisis intervention, and benefits management, ensuring safety, dignity, and quality of life for individuals served. Engagement with families and external providers is essential to success.

Qualifications

• High School Diploma or GED plus 3 years relevant experience, OR an Associate's Degree.
• Minimum two years' experience in human services; supervisory experience preferred.
• Valid driver’s license and reliable car available for all working hours

Responsibilities

• Initiate, support, and ensure quality of life for individuals served through empowerment, safety awareness, respect, and dignity.
• Participate in Multidisciplinary Team meetings and arrange follow-up care to ensure overall health and well-being.
• Oversee all aspects of medication treatment and administration, including MAP processes and MORs.
• Provide crisis intervention and emergency services as needed, including on-call duties.
• Implement ISP goals and strategies while coordinating with staff, providers, and families.
• Ensure documentation in the electronic health record is thorough and timely.

What You Will Do:

• Initiate, support, and ensure quality of life for individuals served through empowerment, safety awareness, respect, and dignity
• Ensure overall health and well‑being of individuals served by participating in Multidisciplinary Team meetings, accompanying individuals to medical appointments, and arranging follow‑up care
• Oversee all aspects of medication treatment and administration, including Health Care Provider Orders, Medication Administration Records, Current Medication Lists, Weekly and Monthly MAP Checklists, and Medication Occurrence Reports (MORs), ensuring corrective action from MAP audits occurs
• Provide crisis intervention and emergency services as needed, including serving as a member of the on‑call team
• Act as a primary implementer of strategies to meet ISP goals
• Take necessary action to protect individuals served in the event of an abuse/neglect complaint and complete required follow‑up from internal and external investigations
• Complete and review documentation within the electronic health record, including work prepared by supervisees, in a thorough and timely manner
• Complete program‑specific and time‑sensitive documents, including Health Encounter Forms and Contract Reports, and ensure proper review before submission
• Prepare documentation for the ISP process, including annual assessments, goals/objectives, support strategies, and progress summaries as required by BHN and DMH
• Ensure communication occurs between staff, the program, outside agencies (including health care providers and pharmacies), and families
• Recognize and report program‑specific safety and maintenance concerns, ensuring property is safe and well maintained, and ensure fire drills are conducted per the Safety Plan
• Ensure individuals served receive benefits management support, including Social Security, health insurance, food stamps, and housing subsidies
• Follow agency policies in alignment with DMH Licensing and Certification requirements

Who You Are:
