Job summary

Behavioral Health Network, Inc (BHN) is seeking a Registered Nurse to join the Adult Community Clinical Services (ACCS) team in Western Massachusetts. The nurse will provide direct and indirect nursing services, oversee MAP practices, and coordinate medical and psychiatric care for individuals served.

Responsibilities include assessments, treatment planning, service delivery, and collaboration with a multidisciplinary team to support person-centered care.

Qualifications

  • Bachelor’s degree required.
  • Current, unrestricted RN license required.
  • MAP knowledge and medication management experience preferred.
  • Strong clinical judgment and communication skills.
  • Experience in community-based nursing preferred.

  • Lead health-care management meetings for preventive and care coordination.
  • Provide in-service training and staff education on medical care and wellness.

What You Will Do
  • Provide direct and indirect nursing services to individuals served by BHN's Adult Community Clinical Services (ACCS) program, collaborating with the multidisciplinary team to support coordinated, person-centered care.
  • Oversee Medication Administration Program (MAP) practices and provide medical and psychiatric monitoring, consultation, and care coordination for individuals served.
  • Participate actively in assessment, treatment planning, and service delivery, including determining the need for rehabilitative services in accordance with applicable Medicaid requirements.
  • Complete nursing assessments, progress notes, health histories, status updates, and self-care sections of evaluation reports in accordance with agency and program requirements.
  • Identify health and nutritional needs and recommend appropriate interventions for inclusion in individuals' person-centered treatment and recovery plans.
  • Serve as a liaison to medical providers and facilities, coordinate appointments and follow-up care, and accompany individuals to appointments when arranged with program leadership.
  • Convene regular health-care management meetings to help ensure individuals receive necessary preventive, primary, specialty, and behavioral-health care.
  • Provide in-service training, consultation, education, and practical support to staff, individuals, and families regarding medical care, wellness, medication, and health-related needs.
  • Use motivational interviewing and recovery-oriented approaches to engage individuals, address ambivalence, promote empowerment and human rights, and strengthen community integration.
  • Maintain timely documentation in CareLogic, Smartsheet, and other required systems; attend meetings and training; identify safety and maintenance concerns; and complete other program-specific duties as assigned.
Who You Are
  • Bachelor's degree in a related field required.
  • Current, unrestricted Registered Nurse license and required nursing certifications required.
  • At least six months of experience in human services preferred.
  • Knowledge of MAP requirements, medication management, physical-health assessment, treatment planning, care coordination, and community-based nursing practice.
  • Strong clinical judgment, interpersonal skills, organization, time management, conflict resolution, teamwork, and verbal and written communication skills.
  • Commitment to recovery, empowerment, human rights, culturally responsive care, and the community integration of individuals with complex behavioral-health needs.
  • Demonstrated ability to work effectively with diverse and challenging populations and to serve individuals and families from varied cultural backgrounds.
  • Working knowledge of community resources and demonstrated ability to help individuals access medical, behavioral-health, social, and recovery supports.
  • Proficiency with electronic records, internet, email, word processing, agency communication systems, CareLogic, and other required technology.
  • Valid driver's license and a reliable, properly registered and inspected vehicle available during working hours required, along with the ability to meet the physical requirements of the role, including lifting or carrying up to 25 pounds, with or without reasonable accommodation.
