Job summary

Behavioral Health Network, Inc. is seeking an Outreach Coordinator for its ACCS team in Western Massachusetts. The role focuses on building recovery-oriented, person-centered relationships and supporting daily living activities to promote independent living.

You will collaborate with a multidisciplinary team, monitor health status, assist with transportation, and document progress in the EHR while adhering to policy and MAP requirements.

Qualifications

• High school diploma or GED required.
• Minimum of six months of human services experience preferred; BHN provides position-specific training.
• Committed to individual empowerment, recovery, human rights, and community integration.
• Strong organizational, interpersonal, and communication skills, with the ability to manage multiple tasks and respond to urgent needs.

Responsibilities

• Build respectful, person-centered relationships that promote recovery, self-determination, choice, dignity, and community inclusion.
• Support individuals with activities of daily living and help them develop and maintain skills for independence.
• Maintain timely documentation in the electronic health record, including progress notes and incident reports.
• Assist with scheduling, transportation, and follow-up for medical, behavioral health, benefits, housing, employment, education, and community appointments.

What You Will Do:

• Build respectful, person-centered relationships that promote recovery, self-determination, choice, dignity, and community inclusion.
• Support individuals with activities of daily living and help them develop and maintain skills needed to live as independently as possible.
• Implement individualized service plan goals and provide timely updates on progress, barriers, and changing needs to the Outreach Coordinator Supervisor and multidisciplinary team.
• Assist with scheduling, transportation, and follow-up for medical, behavioral health, benefits, housing, employment, education, and community appointments—including providing transportation in your own vehicle as needed.
• Monitor the health, mental status, and safety of individuals served, promptly reporting significant changes or risks.
• Assist with medication support and documentation within your training and authorization, in accordance with BHN policy and MAP requirements.
• Provide crisis intervention and emergency support as trained, following required notification and escalation procedures.
• Help individuals build and maintain natural supports, family connections, and community and cultural engagement.
• Support individuals with practical needs such as benefits, identification documents, housing subsidies, budgeting, and referrals.
• Provide hands‑on assistance with household tasks, meal planning, and daily living activities as part of individualized service goals.
• Complete accurate, timely documentation in the electronic health record, including progress notes, incident reports, and Medication Occurrence Reports (MORs).
• Use persistence and creative outreach approaches to maintain engagement with individuals who are difficult to reach.
• Perform other duties as assigned.

Who You Are:
