Job summary

Qualifications

  • High school diploma or GED required.
  • At least six months of experience in human services or related field.

  • Strong problem-solving and crisis-response skills.
  • Good organization, time-management, documentation and follow-through.
  • Valid driver’s license and reliable vehicle required.
  • Ability to lift up to 50 pounds and push/pull up to 100 pounds.

Responsibilities

  • Provide person-centered support promoting safety, recovery and community integration.
  • Assist with activities of daily living, personal care and meal planning.
  • Encourage informed decision-making and self-advocacy.
  • Monitor health and assist with medication documentation per MAP regulations.
  • Provide crisis intervention and communicate changes to the treatment team.
  • Support social networks and participation in community activities.
  • Plan transportation to appointments and community activities.
  • Complete progress notes, incident reports and required documentation.
  • Maintain a safe, clean living environment and report safety concerns.
  • Attend supervision, trainings and maintain certifications.

What You Will Do:

  • Provide direct and indirect person-centered support that promotes safety, recovery, personal growth, independence, and community integration for individuals served through Adult Community Clinical Services (ACCS).
  • Assist with and monitor activities of daily living, personal care, household responsibilities, meal planning and preparation, shopping, and other skills that support greater independence.
  • Encourage informed decision-making, competency, self-advocacy, dignity, and choice while implementing individualized strategies that support ISP/PSTP goals and objectives.
  • Monitor individuals’ health and well-being and assist with medication treatment and administration, including required health and medication documentation, in accordance with MAP regulations and agency policy.
  • Provide crisis intervention and emergency support as needed and promptly communicate changes in health, behavior, safety, or other needs to supervisors and the treatment team.
  • Help individuals strengthen social-support networks, maintain family and guardian relationships, and participate in recreational, religious, and community activities of their choice.
  • Plan and provide meaningful community-based activities and safe, timely transportation to appointments, shopping, errands, recreational activities, residences, and other approved destinations.
  • Complete thorough and timely progress notes, incident reports, medication occurrence reports, periodic reviews, and other program-specific or time-sensitive documentation.
  • Support a safe, clean, welcoming living environment by preparing nutritious meals and snacks, assisting with routine cleaning and maintenance, and reporting safety or environmental concerns promptly.
  • Complete assigned shift responsibilities, including required overnight status calls when applicable; attend supervision, meetings, and mandatory training; maintain required certifications; and perform other assigned duties.

Who You Are:

  • High school diploma or GED required.
  • At least six months of experience in human services, behavioral health, residential services, or community-based support preferred; BHN provides role‑specific training.
  • Commitment to empowerment, recovery, personal growth, human rights, dignity, choice, safety, and meaningful community integration for individuals served.
  • Ability to establish respectful, culturally responsive relationships with individuals from varied backgrounds and support personal-care, daily-living, and community-based needs professionally.
  • Strong interpersonal, verbal, written, observation, problem-solving, and crisis-response skills.
  • Good organization, time-management, prioritization, documentation, and follow-through skills, including the ability to respond appropriately to urgent needs and frequent interruptions.
  • Ability to work collaboratively with individuals, families, guardians, coworkers, supervisors, and multidisciplinary treatment teams while maintaining confidentiality and professional boundaries.
  • Familiarity with internet, email, word-processing, agency communication, and electronic documentation systems.
  • Valid driver’s license and a reliable vehicle available during all working hours required, along with the ability to meet BHN transportation requirements.
  • Ability to work the assigned essential-staff schedule in homes and community settings and meet the physical requirements of the role, including lifting or carrying up to 50 pounds and pushing or pulling up to 100 pounds, with or without reasonable accommodation.
